Ok. Let me try to explain what I have done here, or at least am trying to accomplish. (I'll post SS's of my event boxes)
I'm working on an intro cutscene. I have one event block that serves at the master event, and others that just contain character sprites that the main event uses to move around ect.

Ok, so I'm making an event where an important character and a "demon" have a fight and the character doesn't make it. Therefore I want to use his "damage" sprite to give him the appearance of being felled by his opponent .

So sounds relatively simple, right? Well I must be missing something... Seems like its a hit or miss with the "damage" sprite. I can't seem to consistently get the 'damage' sprite to appear for some reason. Thing is, I have events in other maps that work just fine to this effect. But for some reason, I can't figure out why I can't always reproduce this. I understand that the 'walk' sprites are generally 'standing', 'stepping left' and 'stepping right' with variations of face up, down, left and right, right? I was told at one point on a different site that which foot they standing on (essentially) causes an error with what I'm trying to accomplish. So in short, I'm truly confused about what I'm not getting.

Make sure the character that should have the damage sprite, is set to direction fixed and is facing down. You can set direction fix in the main event or use a move route command to face it down and direction fix before setting to damage sprite.

I have a pretty good guess at how long you been scratching your head lol, I had the same issue a few months back which is why I was able to answer your question. I'm glad its working for you now. Happy game making!
